Through the Chronic Disease Management Plan (CDMP) or Mental Health Care Plan (MHP) through their GP or paediatrician, children can access up to 5 sessions per calendar year. The eligibility criteria include: The child must have a chronic medical condition that has been present for more than 6 months and requires intervention in at least two areas of care.
The child must be under the care of at least two health professionals involved in managing their condition.
